WILL MY COMPANY KNOW MY RESULT?

Your company will not have access to your report, it is strictly private for only yourself to view. Your company will receive a report (if more than 15 employees) of the average results collected from employees who complete the health assessment. Individual cases will not be able to be identified.

DO YOU SHARE MY DATA WITH ANY THIRD PARTIES?

Aggregated and anonymized data will only be used on a research basis by Champion Health to help improve workplace health initiatives. Individual and organizational confidentiality will be fully protected in all instances.

WHAT ARE MY HEALTH REPORT RESULTS BASED ON?

Champion Health pride ourselves on honest and evidence-based information. Your health assessment questions and report are subject to clinically validated questionnaires, research based findings and government guidelines.

IS THERE A USER AGREEMENT LICENSE?

Before accessing you health assessment, you will be asked to read and confirm to the user agreement licence. If you would like to see this before joining the Champion Health community, please find this in the footer of our website. You can see it at: www.championhealth.co.uk/EULA

WHERE CAN I FIND YOUR PRIVACY POLICY?

Once you have completed your health assessment, your health report will be produced immediately.

CAN I CHANGE MY USERNAME?

Usernames cannot be changed. Please ensure you register with your work email address and check it is correct before continuing.

WILL MY DATA BE SECURE?

Data protection is a matter of trust and your trust is important to us. We respect your privacy and personal concerns over data security. Champion Health follow legal provisions strictly as we are committed to compliance with GDPR and national data protection laws. Please read our privacy policy for further information. CAN I CHANGE MY PASSWORD IF I FORGET?

If you are unable to remember your password or would simply like to update it, there is a ‘forgot my password’ option on the login page (www.assessments.championhealth.co.uk) this will send a link to your email to change your password.

CAN I DELETE MY DATA?

If for any reason, you would like to delete any data with Champion Health, please email info@championhealth.co.uk with the data you would like deleted and your reason. Our team will respond and ensure your request is met.

WHAT HAPPENS IF I LEAVE MY COMPANY?

If you decide to leave your company it is the responsibility of yourself and your company to inform Champion Health that you are leaving. We can then discuss if you would like your account and data deleted. After leaving your company, you will still be able to access your account however you will be unable to complete any further assessments.

WHY CAN I NOT GET A COMPANY REPORT?

Champion Health only allow company report access if more 15 or more employees have completed the health assessments. We do this because results can be sensitive for employees therefore we do not want individual cases to be identified.

CAN I JUST HAVE THE SEMINAR, COURSES OR KEYNOTES?

Although Champion Health recommend identifying the needs of your workforce first through health assessments, we do deliver seminars, courses and keynotes alone. However, they may be less effective as you won’t have data underpinning them.

WHAT DO I DO IF MYSELF OR MY EMPLOYEES HAVE ANY PROBLEMS?

HOW LONG CAN I HAVE ACCESS TO YOUR SERVICE?

There is not a fixed amount of time we allow access as each business needs and contract is unique. You can have as little or as much access duration as you require. We advise you including this in your email asking for a quote.

HOW DO I ACCESS THE COMPANY REPORT?

After agreeing a time period for employees to complete their assessments, Champion Health will release the company report after this deadline. You will receive an email notification in which you can then log into your company’s account and access the report.

The price of our service depends on the type of service, amount of employees and the duration of our service. We do subscription models and transactional models to suit all business needs.
