A safety action plan is a document that can help you stay safe when you are finding things difficult or experiencing suicidal thoughts. Download our template below to create your own personal plan.
What do I write in it?
Use your plan to note down ideas on how to get through a difficult situation, ways to keep yourself safe and a list of people you can speak to (including contact details). Examples are included.
When should I complete it?
Safety plans should not be completed when you are in crisis. Instead, they are best written when you have clarity of thought; when you have hope for life or can consider the possibility.
Who should know about it?
Your plan can be just for you or you can share it with someone you trust. This person could also help you to complete it, if you are finding it difficult to come up with ideas.
